Fee for service plan:
Services at Obsidian Health and Wellness, LLC are on a fee for service basis, in which payment is made at the time the service is delivered.
If you have a Health Savings Account (HSA) or Medical Savings Account (MSA) or a Flex Spending Account, you may be able to use this plan to pay for services.
Service Package Plans
For some of our services we give you the option of paying a fixed amount for a "package" of services. For example, we offer a plan for intensive neurofeedback training in the office for one price. You can also combine neurofeedback with hypnotherapy, wellness coaching and heartmath.
Cash, checks, Visa and Master Card credit and debit cards are accepted.
You will need to pay at the time of your visit. When requested we will provide you with a statement that contains all the information you need to submit for possible reimbursement from you insurer.
